Musculoskeletal health is how your body structure enables you to live your life.   Can we help you with any musculoskeletal barrier to health, exercise, and/or movement?  

 

Indika is our Registered Physiotherapist, your musculoskeletal expert, working in the transdisciplinary team, in our new purpose built physio consulting space in the downtown hub.

 

He can help you with pain, injury, post-surgical rehabilitation, arthritis management, respiratory health, neurological conditions, and general bone and joint problems. 

 

Injury work is partially ACC funded and we have several other funded pathways for physiotherapy service.

Acute Care

Introducing Coral, our Extended Care Paramedic, working in the downtown clinic.  

Coral comes to us with 20 years’ experience in the community as a paramedic and now with the additional Extended Care scope of practice.

Extended Care Paramedics assess, treat and refer in the space of acute illness and injuries.

Coral can see people acutely for wounds, strains, sprains, coughs and colds, infections (such as UTIs) rashs, minor burns, abscesses, bites,  skin care, sexual health, gout, ankle or foot injuries, back pain, mild asthma, pneumonia and a long list of other acute conditions. 

 

Working at HHPNZ Coral’s practice in acute care is supported by our broad transdisciplinary clinical team.  

Within her clinical scope, Coral can provide swift assessment and treatment for patients with urgent or acute healthcare needs.

If patients need to see another member of the team, Coral will arrange that onsite as part of her assessment and care.

No repeat scripts – these go to our nurse prescribers please.

Social Work

Our value statement of “a right to health” is central to the way we work.  

 

The goals of the Social Work service are to engage and support our enrolled patient population with a focus on the environmental and social determinants of health.     

​

Our Social Worker practices holistically with a focus on individual and/or whanau support for wrap around services:

  • Advocacy and navigating health systems among other agencies;

  • Resource connection with food, housing, employment, financial, health care among others;

  • Whanau and child support with referrals to support networks as needed

  • Support with parenting, communication and relationships;

  • Community support by bridging the gaps to other agencies and services;

  • Case management with coordination of multiple services;

  • Education on well-being with a holistic approach;

  • Prevention with early intervention to prevent issues worsening;

  • Empowerment with the aim of setting goals and promoting self-advocacy. 

 

Our Social Worker is here to understand you in your context as an individual and/or whanau.   Our Social Worker will walk alongside you through your journey offering realistic solutions with a non-judgemental, culturally sensitive and practical approach while you take the lead and are empowered to make informed decisions.

Our Social Work service is fully funded by us the Health Hub Project NZ and at no charge to you. 

Psychological Skills Group
Support Group

Psychological Skills Group

Ask our hosts or connection team us about our Psychological Skills Group.

 

The group teaches emotional life skills, which are coping skills toward a goal of leading a more satisfying life. The theme of the group is: We are all doing the best we can AND we can still do better. This group is for people experiencing frustration with their emotional state and could include patients who are feeling depressed, anxious, as well as anyone who is ready to make change and learn some new emotional life skills. The group meets weekly on Tuesdays at 1pm.  This is a rolling group which means that a new participant can join during any week of the 8-week group. Each week teaches a new skill with group discussion and activities to go along with it.

 

Participation Criteria:

- Enrolled at HHPNZ

- Must not be experiencing an acute/severe mental health episode.  

- Attendance of 1 individual 30 minute HIP session/pre-group screening session.

- Availability to attend group weekly on Tuesdays from 1-2:30pm.

- Willingness to do brief daily homework and practice skills learned

-Willingness to participate in group activities

 

The sessions include:

Thinking Style Dialectics

Mindfulness

Distress Tolerance

Emotional Regulation

Interpersonal Effectiveness

Addictions and Building a Satisfying Life

 

Feedback on the group has been great so far, with patients showing significant improvements on measures of everyday depression, anxiety and stress.

Persisten Pain Service

Persistent Pain Service

Chronic Pain, also called Persistent or Long-term Pain, affects 1 in 5 New Zealanders.

Chronic Pain is pain that lasts longer than 3 months and can range from mild to severe, consistent or intermittent. Chronic Pain is a complex condition, the symptoms and experiences of which can vary from person to person.

We provide a collaborative, transdisciplinary chronic pain protocol designed to help individuals manage persistent pain and regain a better quality of life.

​

Our dedicated pain team includes medical professionals, physiotherapists, social workers, and psychologists, working together to provide comprehensive care.

The protocol includes:

  • Pain education and skills training in group settings for connection and support.

  • Up to 4 individual physiotherapy sessions tailored to your needs.

  • Medicine review and education to help manage your treatment plan.

  • Social work support to address barriers to healthcare and social stressors.

  • ​

This program is free of charge, providing the tools and support needed to reduce the impact of pain on your daily life. If chronic pain is affecting you or someone you know, help is available.
